Paper documentation We’re faced with ever-increasing demands for better case documentation, doing more with less… chasing a never-ending need to improve compliance and best-practice in the face of new regulations. The noble procession of Quality Control -> Quality Assurance -> Quality Improvement is what we know we need to orchestrate. Efficiently documenting the details we need while avoiding system redundancy and meeting compliance goals can be, to say the least, a frustrating process.

HI-IQ, designed by SIR volunteers, provides an effective solution.

The Quality Assurance module forms the foundation of a suite of paperless tools designed by your peers to help you record, retrieve and analyze complete case documentation.


Step 1. Record
The Quality Assurance module captures essential documentation details, facilitating compliance with regulatory requirements and removing paperwork from your daily routine. Some of the datapoints captured are:

  • Patient Demographics (Name, DOB, MRN, Sex, Address, Referring Physician)
  • Encounter Details (Date of Service, ASA, Patient Status, Physician Operator, Services Performed)
  • Billing Information (ICD's, CPT's, charge codes)
  • Complications (Complication Type, Severity, Outcome, Notes)
  • Follow-Up Details (Schedule, Notes)

Step 2. Retrieve
Just as important as keeping good records is the ability to access those records on demand. Patient safety is enhanced as the system builds a comprehensive procedural history for each patient, including vital details such as allergies, precautions and risk factors. Visual cues alert the user to important conditions like previous contrast reactions and other complications. The integrated scheduling system facilitates the creation and completion of follow up appointments so that outcomes can be closely monitored.

Step 3. Analyze
Perhaps the most valuable features of HI-IQ are its data analysis tools. Both medical and administrative staff will appreciate the ease with which reports can be generated. HI-IQ eliminates the need to search through piles of paperwork to tally complication rates, procedure volumes, costs and more. Simple search screens and complex pre-formatted reports facilitate the retrieval of information from your HI-IQ database. More complicated queries can be generated through the Advanced Search report. And, if SQL's your thing, tap into the power of HI-IQ's built-in SQL engine.

  • Service Activity Report - This report generates a tally of procedure volumes for a given date range. The report also includes success and failure rates, and can be filtered by various parameters (physician, hospital, room, and so on) to allow comparisons.
  • Complications by Service - Complication rates, per procedure are provided in this report. Major and minor complication rates are called out and SIR complication thresholds are listed, where available, to allow benchmarking.
  • CPT/RVU Analysis - Your procedure volume might be higher than ever, but are you really bringing in more money? This report analyzes not just CPT frequency, but also the relative amount of "work" being done so that you can more easily justify increased resources. A favorite with department administrators.
